How to apply for Disabled Facilities Grant

For applicant's under 18 years old

Your child or young person will need an assessment first to see if you are eligible for support from us.

For applicant’s over 18 years old  

Contact our Health and Social Care Hub and ask for a referral to Adult Social Care for an occupational therapy assessment.

How the grant will be paid

We will pay the contractor either in installments as the work progresses or in full when the work is finished.
