Pathway Plan
From the age of 16 your care plan is replaced with a pathway plan. A pathway plan sets out:
- Your views
- Future goals
- What your needs are
- Support you will receive from us
The Pathway Plan is your plan, and you will work together with your PA to support your goals.
We will talk with you regularly to review and update your Pathway Plan. We will review your Pathway Plan at least twice a year, and more often if there are big changes in your life.
Care Plan
Your care plan is written by your social worker and has information about you, your placements, and what your plans might be for the future. It is important that you take part in writing your plan. It should be updated regularly as things change.
Pathway Plan
A pathway plan is your plan and is a written document that looks at your plans and goals and what is needed to help you achieve them. It is important that you take part in writing your plan. This helps identify your current skills and any new skills that you need to develop to help you move towards being more independent.
