The day you leave hospital

Going back to your home

The hospital will make sure that:   

  • Your GP is notified in writing. 

  • Transport is arranged to get you home. 

  • You have any medication or other supplies you’ll need. 

  • You’ve been shown how to use any equipment, aids or adaptations needed. 

  • You have appropriate clothes to wear. 

  • You have money and keys for your home. 

Moving to a care home

Following your assessment if it has been agreed you will move to a care home. The discharge to your chosen care home will take place.

Private funder

If you are a private funder then the Self Funder Advisor within the Hospital Liaison team can provide information on care home and care agencies.

Home from hospital

The Age UK Home from hospital service provides practical support such as picking up shopping, turning the heating and lights on for when you arrive home. As well as advice and reassurance about how to manage at home.
