Short-Term Assessment and Reablement Team (START) is an intermediate care service providing short-term interventions of up to 6 weeks, to promote independence to residents in their own homes across Wokingham. This is a busy service, focused on providing timely rehabilitation goals to achieve maximum independence for the customer using a therapy-based approach.If you are thinking to choose this career as a new career or have experience of working as a Support worker, Care Worker, Healthcare Assistant, or have worked in Domiciliary Care, and are looking to make a difference in our community, we would love to hear from you.To succeed in this role, you will need to demonstrate your values which should match with Optalis core values.
