One.

b) Of these reviews, how many were initiated because of the death of a child, and in what year did that death occur?
Child died in January 2021, Child Safeguarding Practice Review (CSPR) initiated on the perpetrator.

2. Can you please provide the same data requested in questions 1 and 1a for the following time periods:

a) Between 1st January 2017 and 31st December 2017: None.
b) Between 1st January 2018 and 31st December 2018: None.
c) Between 1st January 2019 and 31st December 2019: None.
d) Between 1st January 2020 and 31st December 2020: Two, neither initiated following a child death.
