1. Number of initial requests for new EHC plans (education, health and care plans) on a monthly basis (Jan-2018 to Jan-2021) by source (e.g., mainstream schools, parents, LAs, etc.)

2. Number of children and young people assessed following requests for EHC plans on a monthly basis (Jan-2018 to Jan-2021)

3. Number of new EHC plans issued on a monthly basis (Jan-2018 to Jan-2021)

4. Proportion of EHC plans issued within 20 weeks among all the EHC plans on a monthly basis (Jan-2018 to Jan-2021)

5. Number of annual reviews for existing EHC plans undertaken on a monthly basis (Jan-2018 to Jan-2021)

6. Number of placements made for pupils with EHCPs on a monthly basis (Jan-2018 to Jan-2021)
a) Split the placements by school types, i.e. mainstream schools, maintained special schools, and independent and non-maintained special schools
b) Split the placements by pupil’s age
c) Split the placements by annual fee range, i.e. below 20k, 20-50k, 50-80k, 80k and above

7. Number of pupils with EHCPs who transferred from mainstream settings to independent special schools on a monthly basis (Jan-2018 to Jan-2021)
