1. Do you have a web-based directory of services/e-marketplace for ASC? If yes:
i. Is this developed in-house or by an external provider?
ii. If an externally, please provide the name of the supplier?
iii. If an external provider, please provide the renewal date for the contract.
iv. What is the annual support cost for your directory/eMarketplace?
v. Are there any plans to review provision of your directory in the next 12 months?
2. Please provide the name(s), email(s) of the individual responsible for the directory/e-marketplace at the LA?
3. Who oversees the management of personal budgets, direct payments, what are their contact details? – i.e. name(s), email(s)?
4. Does the LA provide an in-house managed account service for personal budgets?
5. Does the LA offer the option of ISFs, if so, who oversees this, what are their contact details?
6. Does the LA support admin, delivery, and or/management of Personal health budgets, e.g. on behalf of the NHS Integrated Care Board, if so, who is the contact at the LA for this their contact details?
7. Do the LA use 3rd party solutions to assist with delivery and budget management of personal budgets, direct payments, or integrated budgets? E.g. pre-paid cards, eWallet solutions, direct payment support services, digital software?
8. If yes, what solution is used, who is the provider and when is the contract due to expire?
9. If no, is this being considered, who in the LA is the person to contact about this, what are their contact details - i.e. name(s), email?
10. Does the LA have a Personal Assistants register, if so, who is the person at the LA who is responsible and their contact details - i.e. name(s), email(s)?
11. Who is the Director of ASC, what are their contact details i.e. name(s), email(s)?
12. Who is the Assistant Director of ASC, what are their contact details i.e. name(s), email?
13. Who oversees joined up working with the LA and ICB/ICS (if applicable) and what are their contact details - i.e. name(s), email?
