I am writing to request information under the Freedom of Information Act 2000. In order to help you with this request, I am outlining my query as specifically as possible. Please can you provide me with the following information held by your council regarding Disabled Facilities Grants -

1. The Head/Manager responsible for Disabled Facilities Grants, their contact details including telephone number and email address.

2. Details of your current software that you use to manage customers applications, e.g. NEC, Idox, Civica, etc. including contract cost, duration of the contract and its expiry date.

Disabled Facilities Grants help disabled people towards the cost of making changes to their home so they can continue to live there.

Software – Foundations Case Manager currently being implemented.
Contract cost - £3,600 per annum
Duration of Contract – rolling annual contract
