Care leavers with SEND
Support for young people and young adults with special educational needs and disabilities
Councils in England are required to provide information to children and young people aged up to 25 years with special educational needs and disabilities (SEND) and their families about:
- Education
- Health and care services,
- Leisure activities
- Support groups
This is known as the SEND Local Offer. It helps parents, carers, and young people understand what they can expect from local services and how to access them.
You can use the Local Offer to:
- Find out what to do if you need more support
- Get advice and support for to prepare for adulthood
Help from your Personal Advisor
Your PA can give you advice and help you with:
- Participating in planning
- Making choices
- Developing independence and your life skills
- Transitioning from children services to adult services.
Wokingham SENDIASS
Wokingham SENDIASS can provide young people with SEND up to the age of 25 and their parents/carers with free, confidential, impartial advice, guidance and support relating to all aspects of SEN and Disabilities, including:
- Health
- Social Care
- Personal Budgets.
